Someday I should put these instructions in the wiki, but for now, I will use the Forum. The link is to the Middlesex Collection, 3rd ed. published in 1811. This hymnal isn't in Hymnary, so you will need to add it. The link to add a hymnal is on the left side under Editor Tools - Add hymnal. Make up a hymnal ID using the initials in the title words and the year, which would be something like MCCM1811, then fill in the other information.

You may upload the hymn information using a spreadsheet and then connect all the authorities after you upload it, or you can add the hymn information directly using the form under Editor Tools - Add new - Hymn text instance, which is the way I prefer to do it. You put in your created hymnal ID, then add as much other information as you can. Once you have the text instance added, you can add a tune to it. It looks like the hymns are numbered by page number.

To upload the page scans, from the Internet Archive page, you click on the left where it says "View this book" on the link that says "All Files - HTTPS" Look for the file that ends with "jp2.zip, click on it and it should download. Then, in your hymnal, where it says "Editor Tools" there is a link that says "Upload page images." On the upload page images form, where it says "Introductory Pages" count the number of pages before page 1, including the cover, and select that number. (The first numbered page in this hymnal is 11, so you would count back until you get to 1, then count the number of pages before 1). Check the box that says zip file includes cover image. This should make the page numbering match the hymnal numbers. Select your file, then click the upload button. It will take a few hours for the images to convert.

To attach the images, you should see a link on your hymnal page that says "Attach images to this hymnal." It is pretty intuitive. You skip the pages before the first hymn, then, since the hymns are numbered by page number, you type in "11" for the first one, then you just tell the program which hymns are on the page.
